These Terms & Conditions govern use of the Holistic Origin Her WellnessTM website and, where applicable, the
administrative terms associated with scheduling and receiving consultation services. Clinical consent, Telehealth
consent, privacy practices, financial obligations, and cancellation requirements may also be governed by separate
practice policies and consent documents.
By using this website or scheduling consultation services, you acknowledge these Terms & Conditions. If you do not
agree with the website terms, do not use the website. If you have questions about terms that apply to consultation
services, contact the practice before scheduling or receiving services.
ABOUT HOLISTIC ORIGIN HER WELLNESSTM
Holistic Origin Her WellnessTM is a Telehealth-based women’s health practice owned and operated by Feonie Eleuterio,
FNP-BC. The practice provides personalized consultation services to adult women age 21 and older who are physically
located in a state where the provider is authorized to provide the applicable services.
Current consultation areas include:
• Metabolic & Preventive Wellness
• Reproductive Wellness & Fertility Planning
• Contraceptive education and reproductive planning
• Preconception wellness
• Menopause & Midlife Wellness
• Nutrition and lifestyle wellness
• Healthy aging and preventive-health priorities
Holistic Origin Her WellnessTM is a telehealth-only practice and does not provide in-person healthcare services.
WEBSITE INFORMATION IS EDUCATIONAL
Information available on this website, including articles, guides, checklists, downloadable resources, and other
educational materials, is provided for general educational and informational purposes.
Do not use website information as a substitute for individualized evaluation by an appropriate healthcare professional.
Do not start, stop, or change prescribed medication solely on the basis of general website or educational content.
CONSULTATION SERVICES
Holistic Origin Her WellnessTM provides individualized women’s health consultation services through telehealth.
Consultation services may include review of health history and concerns, review of relevant laboratory results and
imaging reports, evidence-based education, preventive-health guidance, reproductive and menopause-related
guidance, nutrition and lifestyle recommendations when appropriate, and recommendations regarding appropriate next
steps.
The scope of each consultation depends on the patient’s health history, concerns, available information, clinical
circumstances, and the provider’s professional judgment and scope of practice.
Consultation services are governed by the practice’s Telehealth Informed Consent & Consent for Consultation Services
and other applicable patient policies.
PROVIDER-PATIENT RELATIONSHIP
Use of the website, submission of a general contact form, downloading educational resources, or requesting information
does not by itself establish a provider-patient relationship.
A provider-patient relationship for consultation services is established only when applicable intake and consent
requirements have been completed, the provider has accepted the patient for consultation, and a professional
consultation is initiated or otherwise provided in accordance with applicable law.

The existence, scope, and duration of any provider-patient relationship are subject to applicable professional and state
requirements.
CONSULTATION-ONLY MODEL
The practice does not provide ongoing medication or prescription management as part of its consultation services.
When medication treatment, medication changes, diagnostic testing, procedures, in-person examination, or specialty
evaluation may be appropriate, patients may be advised to discuss those recommendations with an appropriate treating
healthcare professional.
Patients remain responsible for maintaining appropriate ongoing medical care and for seeking outside evaluation or
treatment when recommended or necessary.
TELEHEALTH SERVICES
Consultation services are delivered through Telehealth technology. Telehealth has inherent limitations, including the
inability to perform a complete hands-on physical examination and the possibility of technology or connectivity
problems.
Certain concerns may require in-person evaluation, diagnostic testing, specialist consultation, urgent care, emergency
care, or follow-up with an established healthcare professional.
Patients must review and complete the practice’s Telehealth Informed Consent & Consent for Consultation Services
before receiving consultation services when required.
STATE LICENSURE & PATIENT LOCATION
Consultation services are available only when the patient is physically located in a state where the provider is
authorized to provide the applicable services.
Holistic Origin Her WellnessTM currently serves eligible patients physically located in New York, New Jersey, and
Connecticut. Patients must accurately report their physical location at each telehealth consultation.
An appointment may need to be rescheduled if the patient is outside an eligible state at the scheduled consultation time.
EMERGENCY & URGENT MEDICAL CONDITIONS
The website, patient portal, email, and other practice communication channels should not be used for medical
emergencies or conditions requiring immediate attention.
If you believe you are experiencing a medical emergency, seek immediate emergency medical care through the
appropriate emergency service in your location.
PATIENT RESPONSIBILITIES
Patients receiving consultation services agree to:
• Provide accurate and complete health and contact information to the best of their knowledge
• Provide accurate physical-location information during telehealth consultations
• Communicate relevant symptoms, health history, medications, supplements, allergies, and concerns
• Ask questions when information or recommendations are not understood
• Maintain appropriate ongoing care with treating healthcare professionals
• Seek in-person, urgent, or emergency care when recommended or necessary
• Use patient-portal and practice communications appropriately
• Comply with applicable scheduling, financial, cancellation, and privacy policies
Incomplete or inaccurate information may limit the provider’s ability to provide appropriate consultation guidance.
APPOINTMENTS, CANCELLATIONS & RESCHEDULING
Appointment scheduling, cancellation, rescheduling, late-arrival, and no-show requirements are governed by the
practice’s current Cancellation & Rescheduling Policy.
Holistic Origin Her WellnessTM provides consultation services and does not replace a patient’s primary care
provider, OB-GYN, specialist, or other treating healthcare professional.
Holistic Origin Her WellnessTM does not provide emergency medical services, urgent-care services, or continuous
patient monitoring.

Patients are responsible for reviewing and complying with the policy in effect at the time of scheduling.
PAYMENT & CASH-PAY SERVICES
Patients are responsible for consultation fees in accordance with the practice’s current Financial Policy. Payment
requirements, accepted payment methods, refunds, and other financial terms are governed by that policy.
Laboratory testing, imaging, medications, specialist services, procedures, and other services provided outside Holistic
Origin Her WellnessTM are not included in the consultation fee and may result in separate charges.
Insurance coverage for outside services depends on the patient’s individual insurance plan and the outside provider,
laboratory, pharmacy, facility, or other organization providing the service.
GOOD FAITH ESTIMATE
Patients who are uninsured or who choose not to use insurance for applicable healthcare services may have rights
under federal law to receive a Good Faith Estimate of expected charges when required.
Additional information is provided in the practice’s Good Faith Estimate notice and related financial materials.
REFUNDS
Fees for consultation services that have already been provided are generally non-refundable, subject to applicable law
and the practice’s Financial Policy.
Fees associated with canceled, rescheduled, missed, or unused appointments are governed by the current Financial
Policy and Cancellation & Rescheduling Policy.
PATIENT PORTAL & ELECTRONIC COMMUNICATION
The patient portal and other approved practice communication channels are intended for administrative communication
and non-urgent matters related to consultation services.
Messages are not continuously monitored. Response times may vary based on business hours, provider availability,
weekends, holidays, message volume, and the nature of the request.
Electronic communication does not constitute continuous monitoring or unlimited access to the provider.
OUTSIDE SERVICES & REFERRALS
The provider may recommend that a patient seek testing, in-person evaluation, or care from another healthcare
professional or facility when appropriate.
Unless otherwise specifically arranged, patients are responsible for establishing care with outside healthcare
professionals, scheduling outside services, and confirming any associated costs or insurance coverage.
Holistic Origin Her WellnessTM is not responsible for the independent clinical decisions, billing practices, availability, or
services of unaffiliated outside providers or facilities.
WOMEN’S HEALTH ROADMAPTM & STRATEGY TOOLKITTM
Eligible consultation services may include a personalized Women’s Health RoadmapTM, Women’s Health Strategy
ToolkitTM, consultation materials, or educational resources.
These materials are intended to support the patient’s understanding of consultation findings, priorities, questions, and
recommended next steps. They do not replace emergency care, in-person physical examination, diagnostic testing
when indicated, or ongoing care from treating healthcare professionals.
NO GUARANTEE OF RESULTS
Health outcomes vary among individuals. Holistic Origin Her WellnessTM does not guarantee a particular clinical
outcome, symptom response, fertility outcome, weight change, laboratory result, or other health result.
Recommendations are based on the information available at the time of consultation, current evidence, and the
provider’s professional judgment. New information or subsequent evaluation may change what is appropriate.
RIGHT TO DECLINE OR DISCONTINUE CONSULTATION SERVICES
Holistic Origin Her WellnessTM may decline to initiate or may discontinue consultation services when clinically,
professionally, operationally, legally, or safety-wise appropriate, subject to applicable law and professional obligations.
Holistic Origin Her WellnessTM operates as a cash-pay consultation practice and does not bill insurance for
consultation services.

Reasons may include, as permitted by law, requests outside the provider’s scope or consultation model, inability to
safely provide services through telehealth, repeated non-payment or missed appointments, abusive or threatening
behavior, misuse of practice services, or regulatory requirements.
When legally or professionally required, appropriate notice or transition information will be provided.
INTELLECTUAL PROPERTY
Unless otherwise indicated, website content and practice-created materials are owned by or licensed to Holistic Origin
Her WellnessTM and are protected by applicable intellectual-property laws.
Protected materials may include:

• Graphics and design elements

Because consultation services may be provided to patients physically located in New York, New Jersey, or Connecticut,
the laws and professional requirements applicable to a particular consultation may depend on the patient’s location and
other circumstances.
This limitation is intended for website and general terms purposes and does not eliminate professional obligations
applicable to consultation services.
